Contents: Concha bullosa pyocele is an uncommon disorder and occurs when concha bullosa becomes infected. In this study, a case of massive concha bullosa in a 30-year old woman, who had been complained about nasal obstruc-tion and headache for 4 years was reported. Anterior nasal examination revealed a massive mass on right side. Computed tomography of the paranasal sinuses showed a septal deviation and a right intranasal mass without causing bone destruction at neighboring struc-tures. Under general anesthesia, partial middle turbinate resection was performed by transnasal endoscopic technique. No complication occurred during or after op-eration. At follow up examinations no complain was noti-fied by patient and nasal passage of patient was found to completely open. Computerized tomography following 6 months of intervention revealed no intranasal lesion or mass. The case was presented in order to attract atten-tion to intranasal concha bullosa pyocele in the diferen-tial diagnosis of patients with nasal obstruction and headache.
