Although humoral antibody mediated mechanisms play a role in liver allograft rejection, no data exist concerning B-lymphocytes and plasma cells. The aim of this study was to analyse whether 1) B-lymphocytes and plasma cells are present in biopsies of acute graft rejection. 2) B-lymphocytes and plasma cells are expanded by local proliferation or immigration and 3) whether the B-cell specific chemokine MIP-3a /CCR-6 is expressed in liver tissue. Biopsies of 20 patients with acute graft rejection (AR-biopsies) were analysed by immunohistochemistry (CD20, CD38, CD 138, CD 20/Ki67, MIP-3a , CCR-6) and by RT-PCR (MIP-3a ,CCR-6). In all biopsies with acute graft rejection (RAI-Score 1-9) CD20+ B-lymphocytes and CD38+/138+ plasma cells could be detected, MIP-3a or CCR-6 expression could be observed in all AR-biopsies on granulocytes and mononuclear cells, which was confirmed by RT-PCR. In comparison of AR-biopsies with AO-biopsies (biopsies taken before transplantation) there was a significant increase in the quantity of CD20+ B-lymphocytes (p=0,002) and CD38+/CD138+ plasma cells (p=0,022) whereas no significant differences could be observed in the quantity of proliferating B-lymphocytes (p=0,262). These data suggest that local B-lymphocyte/plasma cell expansion occurs not by proliferation but by immigration. The local expression of MIP-3a and CCR-6 indicates that this chemokine is involved in B-cell accumulation during acute graft rejection and suggests a local, antibody mediated pathomechanism in acute liver allograft rejection.
