Contents: A proposed cardiovascular-disease (CVD) risk-reduction program for United States Air Force (USAF) personnel, the HEART Program, is discussed. We are interested in determining an implementation sequence of bases for the USAF-wide introduction of the HEART Program. A nonlinear 0-1 integer programming model which considers CVD risk, CVD cost, rated flyers, and interbase transfers in choosing a start-up sequence for 118 USAF bases is presented. Heuristic methods for simplifying the model are presented, as are four solutions.
