During biliary surgery, one of the most dramatic complications are probably injuries to the biliary and vascular structures. Studies on anatomical variations are insufficient due to the fact that the patient who will get surgery can present any one of them, or even an own variant. Dissections on cadavers and during surgery have been made to identify the vascular fascia of the cystic artery, noting that on the plane between the fascia and the gallbladder there are no important elements. In contrast, arterial and biliary ramifications are established between this fascia and the lower side of the liver. The aim of this work is to describe the technique for working on the subfascial plane, which is a safe way to avoid unnoticed noble elements injury. |
