Cardiac allograft vasculopathy (CAV) is an important risk factor for long-term morbidity and mortality in cardiac transplant recipients. The incidence is approximately 10% in the first postoperative year and increases to 30-40% within 5 years of cardiac transplantation. The present review article summarizes different immunological and non-immunological factors that contribute to the pathogenesis of CAV. In the chapter 'diagnostics', we describe various methods and their limitations. Then, we present data on the incidence of CAV in our cohort of cardiac transplant recipients, including data in those patients who had received a ventricular assist device implantation as bridge to transplant. In a further chapter, we describe the very restricted therapeutic measures and elucidate several medical treatments with respect to CAV prevention. Finally, we give an outlook about various possibilities to minimize risk factors for CAV with the use of a new perfusion system that is able to transport the beating donor heart to a cardiac transplant recipient. |
