The number and percentage of older hospital patients is increasing. Due to Medicare cost containment strategies, however, older persons are being hospitalized for fewer days and have greater levels of need at discharge. This phenomenon represents a challenge to both hospital and community-based personnel who are expected to provide quality care to the elderly. This article describes a joint project developed by an acute care hospital and a community care agency to effect formal linkages between the systems in order to provide continuity of care to elders and their caregivers. Products of the project include a resource guide to community-based services, a training curriculum to accompany the guide, and a plan for implementation. This project can easily be replicated by other acute and community care providers. |
