This article describes an education model, or teaching tool, designed to assist in training physicians, health care professionals, and lay people in the unique aspects of medical coordination of chronic care. It was developed in response to the inadequacy of existing care algorithms to address the multiple diagnoses and longitudinal care aspects of chronic disease. A case study is presented which illustrates these cycles of chronic care. The teaching tool has been used to present the concepts of longitudinal primary care, to introduce the concepts of team care, and to teach the skill of care coordination. |
