Inhoud: Ethical issues about how people should live out their old age are complicated by multiple factors. When an older person expresses the desire to stay in comfortable familiar surroundings, families, health-care services, and community resources often help to determine whether or not this hope can be realized even if an illness occurs. Unfortunately, too often, the older client is not the one who decides. Decisions are often based, out of genuine concern for the person, on the best judgments of those who deliver the health care or by family members. This genuine concern is insufficient reason for imposing decisions that are not of the client's choosing. How can nurses who are faced with these situations provide care that will help to empower their client's self-determination? Ethical reasoning is the basis for this type of care. In this case study, a nurse uses ethical guidelines to decide what course of action should be followed when the client's wishes are different from the family's wishes.
