Contents: We investigated age trends and specificity of response in children's dental fears. Participants were 50 Black 3- to 11-year-olds from families of low socioeconomic status. Heart rate, observed behavior, and self-reported fear were measured at each of nine treatment phases during routine dental restorations at a community clinic. These nine phases were categorized as (a) highly invasive, (b) intermediately invasive, or (c) noninvasive. Multivariate analyses indicated higher levels of fear during highly invasive phases than during intermediately invasive phases and higher levels of fear during intermediately invasive phases than during noninvasive phases. These effects could not be accounted for by the serial position of phases in the treatment Sequence. Because these effects occurred on all dependent measures, studies that operationalize fear as an average across treatment phases may be misleading. Age differences in fear were most pronounced during the intermediately invasive phases. Results suggest the importance of cognitive developmental status in children's appraisal of ambiguous events; further research is proposed to investigate the cognitive processing implications of this interpretation.
