Contents: Behavioral treatment of headaches has emphasized biofeedback and relaxation interventions, and has been primarily directed towards adults. However, the consequences of pain-reporting behavior may play a role in the development and maintenance of headache reporting behavior, especially among children. This single-case study describes the successful application of a contingency management procedure for the treatment of a six-year old female with a diagnosis of infantile migraine. Using a multiple-baseline across settings design, intervention was implemented in two phases: first at school and then at home. The results indicated a substantial reduction in reported headache pain over the 18 weeks of the study with continued maintenance of effects at a 10 month follow-up assessment. It was suggested that the consequences of headache reports should become a standard component in the assessment of headaches in order to maximize treatment benefits to patients.
