In 1998 the Secretary of State for Health described the apparent failure of 'Care in the Community' (Department of Health, 1998). Distribution of the modernization fund for services to protect the public from psychiatric patients with the potential to behave dangerously will begin soon. A recent review of service developments (Exworthy, 1998) confirms that the current literature concentrates on maximum- and medium-security hospitals where most forensic psychiatrists are based. Exworthy drew attention to the need for health authorities to 'accept responsibility for their patients'. This paper describes the attempts by one NHS trust (York) to create a psychiatric service for its mentally disordered offender patients who do not require confinement in a medium-security setting. |
