Transition Services is a graduate nursing program in the Department of Community Health Care Systems at the University of Washington's School of Nursing. The program was developed as a specific model of health care for preparing nurses to deliver personalized care to people with advanced cancer and their families. One important aspect of the graduate program is to prepare nurses who are knowledgeable and skillfull at providing terminal care to families in their homes. The article describes the clinical content of the program that helps graduate students to learn about the process of dying during three critical periods: before death, the death event, and bereavement. |
